This program provides a brief overview of current anticoagulants, including drug characteristics, therapeutic uses, clinical monitoring, drug interactions, use in special patient populations (e.g., patients with severe renal impairment), and limitations. The clinical use and safety of emerging anticoagulants are addressed, with focus on implications for future clinical practice and the treatment of venous thromboembolism (VTE). The impact of pharmacist-managed anticoagulation services on patient outcomes and costs are described, including an expanded role of pharmacists in anticoagulation management and the impetus provided by the Joint Commission National Patient Safety Goals (NPSG) to reduce the risk of harm from anticoagulants.
