Michael P. Gulseth is Program Director for Anticoagulation Services at Sanford USD Medical Center in Sioux Falls, SD. Previous to this position, he served as an Assistant Professor of Pharmacy at the University of Minnesota College of Pharmacy in Duluth, Minnesota. Dr. Gulseth received his Doctor of Pharmacy degree in 1999 from North Dakota State University in Fargo, North Dakota. He completed residency training at United Hospital in St. Paul, Minnesota.

Dr. Gulseth is lead author and editor of Managing Anticoagulation Patients in the Hospital: the Inpatient Anticoagulation Service, published in 2007 by the American Society of Health-System Pharmacists.

Dr. Gulseth is a member of the Editorial Advisory Board of the American Journal of Health-System Pharmacy. Dr. Gulseth is an active member of several professional pharmacy organizations, including the American Society of Health-System Pharmacists, the Minnesota Society of Health-System Pharmacists, and the American College of Clinical Pharmacy. His research interests include evaluation of antithrombotic therapies in the inpatient setting and evaluation of the benefits of pharmacist-managed inpatient anticoagulation services.

Michael A. Crouch, Pharm.D., BCPS

Michael A. Crouch is Professor and Chair of the Department of Pharmacy Practice at South Universitys School of Pharmacy in Savannah, Georgia. He received his Bachelor of Science degree in Pharmacy from the University of North Carolina and his Doctor of Pharmacy degree from the Medical University of South Carolina. He also completed two years of postgraduate training, including a Pharmacy Practice Residency at Wake Forest University Baptist Medical Center and a Medicine/Cardiology Residency at the Medical University of South Carolina. Dr. Crouch is a board-certified pharmacotherapy specialist, who also holds added qualifications in cardiology.

Dr. Crouch has dedicated much of his career to training pharmacy students and residents. He has received several teaching awards related to these activities. In addition to didactic and experiential teaching, he has developed and supported postgraduate training, including a Postgraduate Year Two (PGY2) Cardiovascular Pharmacy Residency program. His teaching research focuses on the use of technology in and out of the classroom. Cardiovascular research interests focus on techniques to improve patient adherence and drug safety, pharmacoepidemiology, and pharmacoeconomics. He has published numerous articles and textbook chapters in these areas. In addition, he serves as the textbook editor for Cardiovascular Pharmacotherapy: A Point-of-Care Guide.

Dr. Crouch is an active member of the American Society of Health-System Pharmacists (ASHP), the American College of Clinical Pharmacy (ACCP), and the American Association of Colleges of Pharmacy (AACP). He serves on several editorial and scholarly boards, including a current appointment on the editorial advisory board of the Annals of Pharmacotherapy. Nationally, he provides numerous invited presentations related to cardiovascular pharmacotherapeutics.

Robert L. Page II, Pharm.D., is an Associate Professor with the Departments of Clinical Pharmacy and Physical/Rehabilitative Medicine at the University of Colorado Denver, Schools of Pharmacy and Medicine (Aurora, CO), and the clinical pharmacy specialist for the Division of Cardiology and Heart Transplantation.

Dr. Page received his Bachelors of Science degree in Biology and Chemistry from Furman University in Greenville, South Carolina; his Bachelors of Science in Pharmacy and Doctor of Pharmacy degrees from the Medical University of South Carolina in Charleston, South Carolina; and completed his specialty residency in pharmacotherapy at the Medical University of South Carolina. He is presently finishing his Masters of Science in Public Health with an emphasis on epidemiology from the new Colorado School of Public Health. He is a Board Certified Pharmacotherapy Specialist with Added Qualifications in Cardiology, a Certified Geriatric Pharmacist, and a Fellow for the following organizations: the American College of Clinical Pharmacy, the American Heart Association (Council on Clinical Cardiology), the American Society of Consultant Pharmacists and the Society of Geriatric Cardiology. Dr. Page has authored several abstracts, journal articles, and book chapters in the areas of cardiovascular pharmacotherapy, as well as, been honored with numerous teaching awards. His research interests consist of drug-drug and drug-herbal interactions, medication adherence, and pharmacotherapy complications in the heart transplant recipient and heart failure patient.
