Activity Description

Acute coronary syndrome (ACS) and venous thromboembolism (VTE) are significant causes of morbidity and mortality for elderly patients. In this web-based symposium, pharmacists will learn about the safe and appropriate use of antithrombotic therapy in elderly patients. Faculty will discuss the application of quality indicators, evidence-based guidelines, and performance measures related to the management of ACS and VTE to elderly patients. Participants will also receive guidance on the medication reconciliation process as a method of ensuring medication safety as patients transition between the acute-care and long-term care settings. Patient case presentations will illustrate appropriate therapy and the application of guidelines to elderly patients.
